EQUAL CREDIT OPPORTUNITY ACT (ECOA) NOTICE AND DISCLOSURE
If your application for business credit is denied, you have the right to receive a written statement of the specific reasons for the denial. You may obtain this statement by submitting a written request within sixty (60) days from the date you are notified of the credit decision. The creditor will provide a written statement of reasons for the denial within thirty (30) days of receiving your request. The Federal Equal Credit Opportunity Act prohibits creditors from discriminating against credit applicants on the basis of race, color, religion, national origin, sex, marital status, age (provided the applicant has the capacity to enter into a binding contract), because all or part of the applicant’s income derives from any public assistance program, or because the applicant has in good faith exercised any right under the Consumer Credit Protection Act. The federal agency that administers compliance with this law is the Federal Trade Commission.
CREDIT AUTHORIZATION
The undersigned individual(s), as principal(s) of and/or guarantor(s) for the applicant, authorize the creditor, its agents, assigns, potential assigns, funding sources, or potential funding sources to obtain and review personal and business credit information from national credit bureaus and other sources in connection with this application, as well as for any update, renewal, extension of credit, or collection of any resulting account. The undersigned further authorize any financial institution or bank to release any information requested in connection with this application. A photocopy or electronic copy of this authorization shall be deemed as valid as the original.
